Evenflo Study: Babies 0-12 mths Bottle Study

Evenflo® would like your feedback! They are in the process of developing a new vent system for their Purely Comfi™ bottle, and are recruiting moms from our Evenflo Mom's Research group to be considered for their research project. If selected to participate in the usage test, you will be paid $100 for your time and complete participation in all parts of the study. Make sure that you complete this form to be entered into their database.

Q1  Do you have a child between the ages of 0-12 months whom you are bottle feeding at least 1-2 times per day?

Q2  Would you be willing to try a bottle with a new venting system?  The bottle is a standard bottle with a new vent system.  The bottle and vent system have been quality tested and are safe for use with your baby.

Q3  Would you be available and willing to use the bottles for 10 consecutive days, record your experiences daily, and participate in the focus group discussion?  The usage period would begin around February 9, 2010, and the focus groups would be conducted on either February 19 or February 22, 2010.

Q4 If selected for participation, which nipple flow should we include with your bottle samples?

Slow Flow (0-3 months)

Medium Flow (3-6 months)

Fast Flow (6+ months)

Participation is limited to 50 participants, so please reply back as soon as possible.

If selected, your participation would require the following: 

• 
  
  The project requires you to commit to using the bottles for 10 days, a minimum of 1 time per day. 
• You would come to Evenflo's offices in Miamisburg, Ohio to pick up the bottles to begin using them for a 10-day period.
• You will be required to record your daily experiences in an easy online diary.  
• After the 10-day usage period, you will come back to Evenflo's offices to participate in a focus group where we will ask you about your experiences using the bottle.  The focus group will take about 2 hours of your time.  
• You would be expected return the bottles after the usage period is finished.  You can simply bring them when you come to the focus group.
• Your agreement (via electronic acceptance) to maintain the confidentiality of any  of Evenflo's proprietary and trade secret information you may receive, learn or observe during your participation in this usage study.
